A new contract for ductwork and systems installation on the north-east and north-west quadrant developments stand extensions at Manchester United has been awarded to Senior Hargreaves. New development work at the ground is extending the stands into the vacant corners, creating new hospitality suites, corporate boxes bars and restaurants with additional kitchens and toilets. Hargreaves was chosen having provided ductwork for the last major stand improvements, five years ago.



The systems installed, include ducted extract and replacement air supply with services routed through air handling units. These include heating and cooling coils, fans, filters, recirculation boxes and heat recovery units. The overall value of the Hargreaves work will be £570,000. Mechanical consulting engineers, Hannan Associates, based in Denton, is responsible for design of the full mechanical, electrical and public health systems and the contract was awarded to Hargreaves by T Jolly Services who are the main mechanical and domestic services contractor.



Filling in the quadrants will increase capacity at Old Trafford from the current 68,000 to 75,000 easing pressure on the club which has capacity crowds at most games. Ventilation with cooling and heating to offset seasonal changes and the effects of high occupancy will ensure comfort at all times in the new areas. In addition to general ventilation, Hargreaves is to supply fire resistant ductwork for kitchen exhaust ventilation and further fire ducts as part of the stairway fire protection scheme.



Stadium work is a speciality for Hargreaves. Besides Manchester United, the company worked on the MEN Arena and Reebok Stadium. They have also worked on several phases of the Wimbledon upgrade and have just completed work at the new Wembley Stadium.

The Case Method



Placing the "Aha" in Education and the "Hmm" in Play

Attorneys in the New Jersey law firm of Pellettieri, Rabstein & Altman who have represented plaintiffs in other defective product, defective drug and pharmaceutical litigations such as Vioxx, Baycol, Phen-Fen and defective joint implants, have decided to represent seriously injured victims of Bextra.



The Food and Drug Administration forced Pfizer Inc. to pull its drug Bextra off the market on April 7, 2005. The Pfizer drug, a cox-2 inhibitor, has already been linked to an increase risk for cardiovascular problems and now may be linked to an increased risk for Stevens-Johnson Syndrome, a rare and potentially fatal skin disease, and coronary artery bypass surgery infections and sternal wound complications.



“Americans pay higher prices, we are told, to ensure drug safety; so Americans must hold drug companies responsible for the promised drug safety.” asserts Joseph Gorman, Esq., an attorney practicing class action and mass tort litigation at Pellettieri, Rabstein & Altman.



“Through victim lawsuits the judicial process helps the agencies of our administrative branch of government enforce patient safety laws.” explains Arthur Penn, Esq., currently chairman of the Pellettieri, Rabstein & Altman class action and mass tort litigation department and formerly Director of the Division of Public Interest Advocacy for the New Jersey Department of the Public Advocate (1974-1978), Assistant Commissioner in the New Jersey Department of Public Advocate (1979-1981) and Member of the New Jersey Supreme Court Committee on Civil Procedure (1978-1981). “When drug companies face the expense and publicity of widespread victim lawsuits, they learn to recalculate the cost-benefit analyses to include more extensive, responsible testing into the side effects of future drugs. You will limit defective drug lawsuits by reducing the number of defective drug victims, and that will only happen by preventing defective drugs from coming to and remaining on the market.”



In related action, the Food and Drug Administration also issued its strongest “black box” warnings for other non-steroidal, anti-inflammatory drugs (NSAID). These medications include Celebrex and over-the-counter products such as Advil, Aleve and Motrin. The warning will indicate that these drugs carry an increased risk of gastric bleeding and heart attacks. Patients, hopefully, will seriously consider these newly indicated side effects.



Bexta is a nonsteroidal anti-inflammatory drug (NSAID), which was approved for use by the FDA in November 2001. It was prescribed for relief of the signs and symptoms of osteoarthritis, adult rheumatoaid arthritis and primary dysmenorrhea (pain associated with menstruation). The recommended dose for the signs and symptoms of arthritis was 10 mg once daily; the recommended dosage for primary dysmenorrhea was 20 mg twice daily, as needed.



A recent meeting by the American Heart Association discussed the possible side effects of Bextra on the heart. It was claimed that Bextra patients undergoing heart surgery could be at increased risk.



The cox-2 inhibitors have been embroiled in controversy since Merck & Co. voluntarily withdrew its blockbuster drug Vioxx from the market last September, after a study showed it doubled patients' risk of heart attack and strokes.



Another study by WellPoint Inc., linked the painkillers (Vioxx, Celebrex and Bextra) to increased cardiovascular risk, further reinforcing findings of other trials that already have sparked concern over the safety of the popular category of drugs.  



That study indicated that Vioxx increased patients' risk of heart attack and stroke by about 20 percent while Bextra increased the risk by 50 percent.



A study submitted by Pfizer to the FDA on November 5, 2004, which included over 1,500 patients treated after undergoing coronary artery bypass graft (CABG), showed increased risk in patients treated with Bextra, compared with those treated with a placebo, of cardiovascular events such as myocardial infarction (heart attack), cerebrovascular incident (stroke), deep vein thrombosis (blood clots in the leg), and pulmonary embolism (blood clot in the lung).



However deep surgical infections and sternal wound complications following coronary artery bypass surgery are also a serious Bextra concern.



In a December 10, 2004 letter from the Vice President and Medical Director of Pfizer Canada to “health care professionals”, the maker of Bextra revised their product monograph (a drug’s clinical profile) to warn doctors that Bextra has led to an increased incidence of cardiovascular/thromboembolic events, deep surgical infections or sternal wound complications. Pfizer Canada, after discussions with Health Canada, the Canadian government agency responsible for drug safety, now officially advises Canadian doctors and pharmacists that Bextra is contraindicated (not recommended) for the treatment of patients for pain following coronary artery bypass surgery (CABG) due to surgical infections and sternal wound complications, as well as heart attacks, strokes and blood clot embolisms.



The FDA advised patients to seek their doctors' advice and emphasized that short-term use of over-the-counter strength versions of the NSAID medicines does not appear to pose the same risks.



“When patients “self-dispense”, or purchase medicines over-the-counter, they may not sufficiently review or may not fully understand the side effects. They are in pain and totally focused on relief. If physicians are more aware of their patients’ use of over-the-counter NSAIDs, or other over-the-counter medicines, then physicians can explain the nature of and monitor these side effects. They can help patients make more informed choices. Just because a drug is offered in over-the-counter strength does not mean that it has no potentially harmful side effects,” says Connie Bowen, RN, a registered nurse working with Pellettieri, Rabstein & Altman.

"In 1992 I took over as manager and a year later St Thomas' merged with Guy's to become one trust. Combining the two switchboard systems of two large hospitals was complicated and became a bit of a nightmare but we managed it and the system now is extremely efficient.



"Before we became one trust we had a microphone tannoy system so we could make announcements and contact individual doctors but technology has moved on so quickly. The whole Trust has also got a lot bigger. When I started we only had 4 telephonists and no computers but we now have 25 and an extremely high-tech computerised switchboard.



"The whole telecommunications department is also much busier now. We do a lot more and we are not just a call centre but a recruitment centre too. In 1987 we had 800 extensions but now we have 8,500!



"We do get some odd calls particularly at night - there are a lot of lonely people out there who just want to ring for a chat.



"One of my busiest days was the day of the hurricane in 1987 when only two of us managed to get in. We were bombarded by staff and patients trying to get through to tell us that they couldn't' get to work or appointments. It was chaos all morning but we coped.



"The NHS is a fantastic employer and helps people progress. There are a lot of other employers where I would not have been able to become a senior manager because I didn't go to university."

Sunderland Teaching Primary Care Trust



Tina started working for Sunderland Teaching Primary Care Trust in 1989 as an auxiliary nurse in the community. She then moved to the NHS Hospice. Then offered chance to be seconded to do nurse training while Trust continue to pay wages. She's doing a three year course on a new scheme - the Accreditation of Work Based Learning where the Trust and the University work together. She did her first year of training at the Hospice while studying part time, and is now full time at Northumbria University for the next two years. She has a contract with the Trust to return to work for them for two years after she qualifies. She found the first year hard work - full time auxiliary, part time student, mother and housewife! Very excited about the opportunities and she's guaranteed a job at the end.



Claire left school with GCSEs, but without sufficient grades to study for nursing, so she applied to be a Nurse Cadet. She has a five year contract with the Trust - 2 years working for the Trust, then 2 years at university, then one year as a qualified nurse at the Trust. She will also study at Northumbria University.



Tina says it's a fight for the computer at home as her son is also studying to be a civil engineer. She's delighted that both her children will have good careers, and is really excited to be furthering her own career at this stage in her life.

In the 1970's, the switchboard was situated in the East Wing Basement, just along from the mortuary. There was a ghost, 'the Grey Lady', that at least one of the ladies had seen walking along the corridor.



The Pater Nosta lift with no doors just went constantly up and over and back down the other side, which we always meant to try, but never did in case we got squashed at the top!



At that time, we had no computers so all our information was kept in small flip files on each switchboard that we updated ourselves. For emergencies or crash calls, we had a large free-standing chrome microphone with a red button to put out the calls to emergency staff.



The Thames Barrier had not been built at that time and one Spring, the Thames was expected to flood. We asked the Duty Officer what we should do as we were in the basement. He said, "Bring your Wellington boots with you and stay at the switchboard. Don't leave your posts"!

There are numerous factors that affect this delicate balancing act, but by breaking the process down and focusing on the minute details of each stage of conception, reproductive medicine researchers have been able to solve many issues that were a mystery to us just a few years ago.

It all starts with an egg



In fertile women, each month one follicle nurtures the growth of a single egg that is released into the fallopian tube then fertilized. However, the process is not quite so simple for the roughly 6 million American women who struggle with infertility. Stimulating ovulation with medication has been standard therapy for ovulatory disorders, but Dr. Leondires explains that research and practice have led to a more targeted approach in recent years. “Today we have an arsenal of different therapy combinations, from those that work best on young, responsive patients to those that are effective with poor responders whose follicle counts are lower than usual,” he says.

When conception occurs naturally, the sperm meets and fertilizes the egg somewhere between the ovaries and the uterus – usually in the fallopian tubes. However, a host of factors can make this process difficult, if not impossible, and that’s where In Vitro Fertilization (IVF) comes in. “Today we can help women with structural abnormalities in the reproductive organs that make it impossible to conceive naturally, as well as men whose sperm quality or quantity make it difficult to achieve fertilization, to conceive through IVF,” Dr. Leondires notes. “Although IVF facilitates fertilization by joining the sperm and egg in the laboratory it does not guarantee fertilization,” Dr. Leondires points out. “Many factors come into play. The sperm must be able to penetrate the egg, then the sperm and egg membranes must merge, and finally, the egg must ‘close’ itself off to other sperm,” he explains. Doctors can also assess any problems that are occurring during the fertilization process, and fix them with a process called Intracytoplasmic Sperm Injection (ICSI). “With ICSI, one healthy sperm is injected directly into the egg, which eliminates challenges associated with membrane penetration or additional sperm entry,” Dr. Leondires adds.



Monitoring the embryo



Once an embryo is created, doctors have new ways to monitor and evaluate its likelihood of survival in the womb. A recent trend is to allow the embryo extra time in the laboratory, enough to develop into a blastocyst. “As a more mature embryo comprising 125-530 cells, a blastocyst may allow improvement in embryo selection for transfer, which could lead to fewer embryos transferred and a lesser chance of a



Multiple gestation, which in and of itself can be a risk for loss of the pregnancy,” Dr. Leondires explains.



In fact, Dr. Leondires urges IVF patients to consider the potential for multiple births seriously when they are asked how many embryos they wish to transfer per attempt. “I encourage my own patients to weigh their diagnosis, the number of cycles they have undergone, and the risks associated with a multiple gestation. ,” he notes.



After the transfer



“Supporting the early pregnancy is another challenge that reproductive medicine experts have been researching for many years, and we have learned that there are probably hundreds of factors that converge perfectly in order to create a successful implantation,” Dr. Leondires says. The good news is that miscarriage rates are no higher for IVF pregnancies than they are for naturally conceived pregnancies. “When an infertility patient successfully conceives, she is carefully monitored, and given hormone replacements that simulate what the body does naturally to support a healthy pregnancy to term,” Dr. Leondires explains.



“Like many other ‘medical mysteries,’ infertility is becoming less and less mysterious as researchers and physicians work to uncover effective new ways to treat this challenging disease,” Dr. Leondires concludes.

An estimated 3,000 former Manufactured Gas Plant (MGP) sites in the United States await cleanup, and protecting local communities from adverse exposure to airborne contaminants is of paramount concern. The Public Place site, also known as the Citizens site, is an 11.5-acre former MGP operated by Brooklyn Union Gas (a National Grid predecessor company) until the early 1960s. “National Grid has informed us they will perform a full evaluation of this technology prior to initiation of site cleanup,” Scotto continued, “and anything to make it more attractive for National Grid to employ this software is a good thing.”



Tim Minnich, president of MSI and co-creator of PICMET, is hopeful that this financial incentive will encourage the natural gas industry to use the open-path technology. “There are about 3,000 MGP sites remaining to be cleaned up in the U. S. alone,” Minnich said. Most of these plants were built in cities and towns in the 19th century to provide gas for heating, cooking, and lighting before the existence of natural gas pipelines. Coal tar is the principal hazard and the source of many volatile air contaminants, some of which are known carcinogens. Minnich estimates that his firm has invested $100,000 to develop PICMET.

3. Record symptoms and health-related activities in a calendar. Having a timeline of symptoms can help doctors identify trends that may impact diagnosis. For example, flu-like symptoms that occur at the same time every year may be an allergy rather than a cold. Migraines clustered around a menstrual cycle may be related to hormonal fluctuations. And more than four ear infections in a year could lead to more aggressive treatment than another round of antibiotics.



4. Journal symptoms as they occur. Don't wait until the doctor's appointment to try and remember every pain or abnormal feeling you've experienced since the last visit. Seemingly unrelated symptoms may prompt the doctor to try different tests or treatments than originally planned. Frequent urination, for example, may indicate a bladder infection. The latest ICSC weekly chain store sales report on October 4, was full of surprises, mostly positive. It eliminates the possibility of a confidence-induced collapse in spending, suggests that retail sales growth for the September will be at least healthy, and suggests increased discounting. However, a number of drags remain and sales growth may decelerate going forward.

Consumers continued to spend at a healthy pace in chain stores in September despite an array of negative factors including record energy prices and hot weather that weighed on fall merchandise demand. Sales were supported by continued job and income growth, borrowing that supplemented cash flow, and easy comparisons to weak sales last year.



Sales were inflated by record gasoline prices. Some chain stores do sell gasoline and those retailers benefited from the increase in prices. Wal-Mart got 0.6 percentage points of its 3.8% growth from higher gasoline prices. Costco got three percentage points of its 11% growth from gasoline prices. The ICSC estimates that this added nearly 0.5 percentage points to same store sales growth in September. They also estimate it added three percentage points to growth at wholesale clubs.



Overall, the sales growth was stronger than expected, with a significant majority of retailers posting sales growth that exceeded expectations. Combined with the surprisingly small decline in auto sales in September, this provides evidence that consumers are not cutting back on their spending beyond what they are forced to do by energy price-induced declines in real income.



Fears that the sharp drop in consumer confidence would be accompanied by a retrenchment in spending clearly seem unfounded. Consumers continued to spend at a healthy pace in chain stores in September despite an array of negative factors including record energy prices and hot weather that weighed on fall merchandise demand. Sales were supported continued job and income growth, borrowing that supplemented cash flow, and easy comparisons to weak sales last year.



The outlook for overall retail sales for the month, to be reported October 14, is for healthy growth. Gasoline station sales likely grew very strongly again as average gasoline prices for the month were up about 40 cents, nearly double the 23 cent in August. Vehicle sales fell modestly and will be a drag on total sales, although a far more modest one than in August. The acceleration in growth in total store sales suggests at least modest growth in core sales. Of course the outlook could be altered by results at building supply stores, grocery stores, restaurants, and some other specialty stores for whom no sales data are yet available. But, building supply and grocery store sales were likely lifted by hurricane-related rebuilding and restocking.
