UNICEF Pakistan in joint collaboration with the Ministry of Health, Government of Pakistan held the first National Health Media Awards to applaud those media initiatives, which have strongly supported the health campaigns launched nationwide. Mr. Amir Jahangir, Chief Executive Officer of SAMAA TV received a special award for Innovations in Health Journalism. Mr. Jahangir is known for being a media person carrying out health related initiatives to improve the overall health indicators situation in Pakistan; the special award was presented for his contribution and efforts towards the eradication of polio across Pakistan.

SAMAA TV, in joint collaboration with, UNICEF Pakistan and the Ministry of Health was the first media channel in Pakistan to establish a Polio Control Cell aimed at acting as an information bridge between the citizens and the polio control authorities. The cell, first of its kind provided instant information on the initiative to callers in the length and breadth of the country besides performing the vital task of identifying missed areas and children. Since its establishment in October 2008, more than 35,000 children have been ensured the vaccination against the polio virus, who otherwise would have been included as the missed targets during the campaigns.

Speaking on the occasion Federal Minister for Health Mir Aijaz Hussain Jakhrani said "The government is committed to protecting the children of Pakistan from the scourge of polio. We need to reach out to every eligible child, which itself is a huge task and can only be accomplished with the collaboration of all stakeholders including the media". Mr. Jakhrani applauded the media for playing a proactive role in waging a war against disease and disability. He especially hailed SAMAA TV efforts for leading from the front in this initiative.

The Polio Control Cell has also received high acknowledgement at the international forums, where Mr. Bill Gates, Co-chair and Trustee of the Bill and Melinda Gates foundation and the Director General World Health Organization, Dr. M. Chan has applauded the SAMAA model as "THE PAKISTAN MODEL" and called it a role model to be replicated in other epidemic developing countries. The model has also been commended at the Stanford University as one of the prime Innovation Journalism case studies for Public-Private Partnership initiative for media and healthcare delivery mechanisms at the 6th Innovation Journalism Conference held in May 2009.

Speaking on the occasion Mr. Amir Jahangir, said that "this award gives a great sense of achievement, however our achievement lies more in the fact that we were among the key players in ensuring a healthier Pakistan. No doubt that given the popularity, coverage and effectiveness of SAMAA News, our support in bringing innovation to the health delivery mechanisms in Pakistan, however, this is just the beginning, we need to continue the same commitment and finish what we set out to achieve - a polio free future for our coming generations."

According to WHO, Afghanistan, India, Nigeria and Pakistan, which together accounted for 1,514 polio cases in 2008, or 91 per cent of all cases. The number of polio cases reported annually has decreased by more than 99 per cent - from 350,000 in 1988 to 1,660 cases in 2008. Pakistan has 118 polio cases during 2008, which is an increase of more than 86 cases since 2007.

Speaking at the award ceremony, Mr. Jahangir urged all the stakeholders to come forward and jointly work for a better and healthier Pakistan. Mr. Jahangir dedicated the Award to the entire SAMAA TV team for making the campaign a success.

Mr. Jahangir also emphasized that Pakistan needs to focus more on improving its Health indicators. Pakistan ranks 116th out of the 134 economies on the Global Competitiveness Index of the World Economic Forum. "The World Economic Forum at Davos ranks the competitiveness of more than 134 nations globally on an annual basis.

In April 2009, the VINNOVA-Stanford Research Center of Innovation Journalism honored Mr. Amir Jahangir as the Program Advisor to the Research Center on Global Media Development and Journalism. There are approximately 27,000 dogs in Whatcom County. The average dog produces ¾ pounds of waste per day. Which means 27,750 lbs. of dog waste per day. That amount of waste can contain 1,241,730,000,000 fecal coliform bacteria.

Dennis Pate, owner of Doo Bandits says, "According to the Washington Department of Ecology, during rains, dog poop melts away and runoff carries it to storm drains, ditches and streams that feed our rivers, lakes and marine waters. The bacteria from dog poop can end up in shellfish, making people who eat those shellfish very sick. The bacteria can also make water unsafe to drink or to swim in. Nutrients from dog poop can also feed the growth of aquatic plants and algae, which, as these decay, use up oxygen in the water that fish and other aquatic life need." Pate would like to remind dog owners to pick up after their pet and carry dog waste bags when they take their dog for a walk.

What Would Jesus Pasteurize?

Mandatory almond pasteurization restricts consumer rights and religious freedom.

As of September 1, 2007, all almonds produced in California, which are destined for the U. S., Canada, and Mexico will be required to undergo pasteurization, or at least that is the ruling passed by the Almond Board of California with the backing of the USDA. Raw foodists around the world depend on a reversal of this ruling and followers of the Essene teachings may face giving up one of their staples.

The basic reason for this exercise in control over citizen's food is two separate incidents of salmonella poisoning found in raw almonds over the last six years. These incidents involved conventional almonds, not organic almonds. Even though no incidents of salmonella poisoning have been reported from organic raw almonds, the Almond Board has decided that they, too, should be pasteurized. So, while there has been no evidence that raw organic almonds are susceptible to salmonella poisoning, the Almond Board is taking away the major source of one of raw foodists' staples.

Another problem with this ruling is that the pasteurized almonds will not be required to be labeled as pasteurized. Consumers may see pasteurized almonds labeled only as “raw” almonds. If the pasteurized almonds are not labeled as “pasteurized”, this will be misleading to consumers and removes the public's ability to make informed decisions.

If there were a lot of other sources of raw almonds, this ruling might not be as serious an issue. However, California happens to be the only U. S. state commercially growing almonds. In addition, California grows about 80% of the world’s almonds, according to the Almond Board of California website.

If this ruling is not reversed, consumers may unwillingly consume pasteurized almonds. Some raw foodists may become ill as a result and they may have to decide between ending a 100% raw diet and ending their consumption of almonds. A better option may be to help the Almond Board “see the light” regarding raw organic almonds. It is not September yet, and consumers need to know both sides of the story.

Citizens of the United States have the freedom to follow the religion of their choice. This may sound strange when discussing the topic of almond pasteurization. Most pasteurization is a form of cooking. There are, in the Essene teachings, quotes from Jesus (Yashua) that state “…prepare not your foods with the fire of death, which kills your foods, your bodies and your souls also.” Many who follow the raw food diet can understand why Jesus would say such a thing. People have experienced improved, even amazing, health as a result of not cooking their food. Because followers of the Essene teachings, which include eating raw foods, may no longer be able to purchase almonds in their raw state, this ruling is actually treading on their religious rights. The Almond Board may be replacing a raw staple with a pasteurized one. If the Essene followers are unable to find other truly raw sources of almonds, they either turn their backs on the teachings or stop eating the almonds. How can this happen in the U. S.?

This aspect of the almond controversy is addressed on a new website. Raw foodists around the world are doing their best to get this ruling reversed. In the mean time, stock up on your raw almonds… while they are still raw.

It is the bane of many elderly folk -- buttocks and lower back pain. Also known as lumbar spinal stenosis, this is when spinal discs undergo wear and tear from natural degeneration. In the process, the spinal canals are narrowed and nerves may be impinged, causing the typical symptoms.

To correct the problem, open surgery or a laminectomy may become necessary to distract the two vertebrae apart and decompress or release pressure on the nerve root. However, for suitable patients, a new procedure involving an implantable device is now available. Not only does it treat the symptoms with improved results but it spares the patient much discomfort and inconvenience.

Developed in Munich, Germany in 2005 and launched in Europe a year later, the In-Space percutaneous interspinous distraction device (http://www. singaporemedicine. com/leadingmedhub/leadingmedhub1.asp) was designed as a tissue-sparing alternative to the laminectomy. The former only requires a small incision of around 2cm compared to 10cm or bigger for the latter.

With the aid of X-ray imaging, a wire is then guided to the space that needs to be widened or distracted. An implant (drum) is then railroaded between the affected spinous processes to widen the space between them. Specially-designed metal 'wings' help keep the implant in place by catching onto the adjacent vertebrae.

Unlike a laminectomy, In-Space does not involve the removal of any bone and damaging of any muscles, thus ensuring that the stability of the spine is not compromised in any way.

Since it is a minimally-invasive procedure, patients will not have to tolerate much pain, excessive bleeding, a long hospital stay, and invariably, enjoy a quicker return to work. Surgery for a single level of vertebrae with a traditional laminectomy would take approximately between two and three hours while the In-Space procedure takes just under an hour to complete.

Dr Razmi Rahmat, Consultant Orthopaedic Surgeon at ParkwayHealth's Mount Elizabeth Hospital (http://www. singaporemedicine. com/hcp/hcp1.asp), is one of a few surgeons in Asia trained to do the procedure, the first of which was carried out in August 2008.

"The most important thing is that this procedure is not done in the spinal canal, so the risk of nerve damage is negligible. There is no direct manipulation of the nerves. In fact, besides doing it accurately under image guidance, we are working very far away from the danger zones," maintains Dr Razmi. 

The suitable patients are usually those between 40 and 70 years of age displaying necessary lumbar spinal stenosis symptoms (http://www. singaporemedicine. com/healthcaredest/sg. asp). Still, not everyone can present as an ideal candidate for In-Space. While leg and buttock pain is a common symptom, the patient needs to feel relief the moment he or she sits down.

Dr Razmi explains, "This is a sign that the degeneration is not severe and is still in an early stage. If the pain is ever-present, then this won't work. A laminectomy will then be necessary to relieve the nerve. Still, I'm confident that although its applications are limited for now, more degenerative spinal conditions will in time, be treatable with In-Space."

The In-Space percutaneous interspinous distraction procedure (http://www. singaporemedicine. com/leadingmedhub/leadingmedhub1.asp) costs approximately US$10,000 for a single level of vertebrae done in Singapore. In the US, the procedure for use of the device is still awaiting approval from the Food and Drug Administration and is expected to cost at least twice as much as in Singapore.

Business Implications

Employer-sponsored health insurance is by far the most important source of health care coverage in the United States. In 2003, approximately 159 million nonelderly U. S. residents received employer-sponsored health insurance. However, access to such coverage has declined since 2001, while the ranks of the uninsured have grown, reaching 45 million in 2003. Almost all covered workers in employer-sponsored health plans receive prescription drug benefits. Approximately 200 million U. S. residents (68% of the population) have these benefits administered by pharmacy benefit management companies (PBMs). Employers want PBMs to be more transparent about the discounts and rebates they negotiate from pharmaceutical companies and to be more active in promoting the use of generics and other inexpensive medicines. To defend themselves against growing criticism, PBMs may have to distance themselves from the pharmaceutical industry.

Employers are likely to adopt increasingly aggressive pharmaceutical cost-containment strategies. Multitier formularies will become more common and more complex, as more and more employers instruct their health plans and PBMs to add fourth and fifth tiers to their formulary designs. Copayments for branded medicines (especially nonpreferred brands) will continue to increase rapidly, and

Coinsurance for the upper tiers of formularies will become more common. Economies of scale afforded by mail-order pharmacy will prompt a growing number of employers to offer mail-order discounts or even to impose mandatory use of mail-order pharmacy. The expiration of patents on many blockbuster drugs in the next several years will stimulate the generics market. With an expanded arsenal of generic drugs available, it will be easier for employers to justify policies such as mandatory use of generics.

Consumerism—the policy of requiring consumers to assume increased economic responsibility for their health care choices and (potentially) to bear a greater proportion of costs—is expected to expand rapidly in the next several years. Employees will have greater freedom to make choices about their health care but might try to cut their costs to avoid incurring high deductibles. Drug manufacturers should seek ways to interact more directly with employers, especially very large organizations. For example, pharmaceutical companies could play a role in employers' disease management programs. Sponsoring health-related Web sites would be another way to support employers. Above all, the pharmaceutical industry needs to combat the perception that it is employers' enemy, incurring enormous costs for little return. It is critical to demonstrate the value of innovative medicines.

ASPCA Names Tim Rickey as Senior Director of Field Investigations & Response

The ASPCA® (The American Society for the Prevention of Cruelty to Animals®) has appointed Tim Rickey as Senior Director of its newly created Field Investigations and Response team, effective January 4, 2010.

New York (Vocus) January 5, 2010

The ASPCA® (The American Society for the Prevention of Cruelty to Animals®) has appointed Tim Rickey as Senior Director of its newly created Field Investigations and Response team, effective January 4, 2010.

Rickey was formerly the Director of the Animal Cruelty Task Force and Disaster Response team for the Humane Society of Missouri (HSMO), where he recently led the largest dog fighting raid in U. S. history, covering eight states and seizing more than 500 dogs in one day.

“Tim Rickey’s unrelenting commitment to fighting animal cruelty is longstanding and will assure that the ASPCA’s resources are coordinated and focused on our mission to prevent cruelty to animals,” said Matt Bershadker, Senior Vice President of the ASPCA’s Anti-Cruelty department. “His courage and tenacity has meant a second chance for countless animals and serves as a giant step forward in the fight against animal cruelty.”

In his new post, Rickey will lead the efforts of the ASPCA’s Field Investigations and Response team, which covers natural disasters as well as large and small animal rescue operations. The team is part of the ASPCA’s Anti-Cruelty group, which is also comprised of Humane Law Enforcement, Government Relations, and Forensic Sciences and Anti-Cruelty Projects.

Rickey’s 20-year career has been devoted to animal welfare issues, first as an animal caretaker at the Joplin (Mo.) Humane Society and as an animal control officer for 11 years at Joplin Animal Control. He joined the HSMO in 2002 as a statewide animal cruelty investigator, and was named director of its Anti Cruelty Task Force and Disaster Response team in 2005.

Rickey led the HSMO’s recovery efforts following Hurricanes Katrina, Rita, Gustav and Ike, major floods in Iowa and multiple Missouri incidents including ice storms, floods and tornados. He also directed field operations for large scale rescues in states throughout the Midwest and Southeast and was instrumental in ensuring that animals are included in statewide disaster planning throughout Missouri.

In 2008, Rickey and his team of investigators provided information that led to an 18-month, multi-state dog fighting investigation with the Missouri Highway Patrol and the FBI. On July 8, 2009, he coordinated animal rescue efforts and crime-scene evidence gathering in five states: Missouri, Illinois, Iowa, Texas, and Oklahoma. Dogs also were rescued in Nebraska, Arkansas and Mississippi. The multi-location evidence and animal recovery operation was assisted by the ASPCA and Humane Society of the United States. Rickey was one of four recipients of the ASPCA’s “Law Enforcement Officer” of the Year award, presented at the ASPCA’s Humane Awards Luncheon on October 29, 2009.

Rickey attended the University of Missouri-Columbia’s National Cruelty Investigation School. He is a frequent instructor on such topics as animal welfare and the law. He is also certified in numerous animal rescue operations, including large animal, swift water and rope rescue, and has completed Department of Homeland Security/FEMA coursework. He earned diplomas in Veterinary Technology, as well as Wildlife, Forestry and Conservation, from Thompson Learning Institute.

Born in Joplin, Mo., Rickey and his wife, Teresa, and their two daughters live near St. Louis, where they tend to their own menagerie, which includes a cat, three dogs, three horses, four goats and 17 chickens—all rescues.

As his governorship turns the corner toward the new election season, Governor Ed Rendell claims Pennsylvania has “turned the corner” on its medical liability crisis. Yet, Pennsylvania remains in crisis. Thousands of physicians have been forced out of business due to skyrocketing medical liability premiums that have resulted from rampant frivolous malpractice lawsuits. Moreover, most student physicians leave Pennsylvania upon completion of their training because they cannot afford to practice here.

“Governor Rendell’s painted scenario might be rosey, but Pennsylvania’s physicians and patients are still stuck by the thorns of reality,” said Politically Active Physicians Association President John Salvo. “An increasing number of patients are losing access to health care because we are losing doctors.”

The governor claims 34,000 physicians pay into the MCARE Fund—which he says proves the crisis is easing. But he fails to disclose of those 34,000, thousands are nurse midwives, medical schoolteachers, hospitals and others.

Governor Rendell has a balancing act he must perform to be reelected: protect the trial lawyers who substantially fund his candidacy and win the powerful voting block of doctors against whom those lawyers litigate. And so, rather than seek meaningful tort reforms such as reasonably limiting jury awards and attorney fees, the governor hides behind the smoke and mirrors of the MCARE bandage.

“Rendell’s campaign to physicians from the back pocket of trial lawyers is an insult to the patients of Pennsylvania who desperately need the tort reforms he has purposely failed to provide,” said Salvo.
