Singapore Raises the Bar with New Orthopedic Technique



An innovative surgeon here has developed a simple yet effective technique to tackle the high failure rates associated with conventional methods of mending Acromioclavicular Joint (ACJ) dislocations.

Dr Lim Yeow Wai readily supports the adage that necessity is the mother of all inventions. He is, after all, a medical innovator in his own right.



A consultant orthopaedic surgeon at Raffles Hospital, Dr Lim pioneered his own method of treating shoulder dislocations after observing that conventional procedures do not offer lasting solutions.



"There have been numerous types of surgeries performed for shoulder dislocation but so far, outcomes from these various methods have not been good, especially when the acromioclavicular joint (ACJ) is involved," he says.



The joint, which is located at the top of the shoulder, is made up of two bones – the clavicle and the acromion – and four ligaments which give it stability. Depending on the severity of a shoulder injury, a person may sprain or tear one or all of the ligaments.



According to Dr Lim, shoulder-related injuries are relatively common in today's athletic society. ACJ injuries are often sustained by cyclists, as well as those who engage in contact sports such as rugby, martial arts and American football. If not properly treated, it can lead to significant loss of strength in the affected limb.



"Presently, most of the treatment methods employ rigid forms of fixation using metal implants such as hook plates, pins and screws. This bolts the bones in place, but often leads to failure because the ACJ joint is not meant to be fixed in place. It is a flexible joint which can rise and rotate during movement. Also, screws and pins are often ineffective because they usually work themselves loose after some time, due to necessary movements in and around the shoulder area," explains Dr Lim.



His dissatisfaction led him to develop the Triple Endobutton technique – a non-rigid method that allows for normal movement of the joint. All that's needed is three tiny buttons and two strands of Fibrewire suture. The buttons are made of stainless steel and are only about 1cm across.



During the operation, the suture is looped through the button holes and over the shoulder bone, resulting in a 'snow shoe' hold that allows for fluid movements while stabilizing the joint. According to Dr Lim, this method gives even greater strength compared to the natural ligaments.



The procedure takes place under general anaesthesia with the patient in a beach chair position. So far, Dr Lim has performed 30 such operations with only one failure, giving this technique a failure rate of less than five percent. This is a far cry from the 20 percent failure rate associated with conventional treatment method.



One patient who has benefitted from Dr Lim's innovative treatment is Mr John Tuffin. The 62-year-old Australian is an avid sportsman who injured his shoulder following a horse riding accident in April last year (2009). To make matters worse, he tripped and fell several months later while boarding a ferry.



In terrible pain, Mr Tuffin sought help at Raffles Hospital and was referred to Dr Lim, who confirmed the diagnosis of chronic ACJ dislocation and explained the Triple Endobutton procedure to a much relieved Mr Tuffin.



"It made good sense to me. I said 'That's it! I'll get it done!' and didn't think twice." The surgery took one and a half hours and he was discharged the day after.



"Although I was in slight pain for the first few days, I saw the benefits immediately. My sleep is no longer disrupted and rolling over in bed does not hurt anymore," says a delighted Mr Tuffin.



All in praise of Dr Lim's efficiency, he says: "Dr Lim was very thorough and did the relevant checks to ensure that my body was fit for the operation to be carried out. He is a very good surgeon and I have much faith in his hands. The results more than speak for themselves."



"My stay at Raffles Hospital was excellent! All my expectations were met and I don't think the doctors and nurses could have done anything more. Excellent!"



Currently, the Triple Endobutton technique is only performed in Singapore. The cost of the surgery is S$10,000. This compares favourably to conventional shoulder surgeries in the US, which usually range between US$20,000 and US$25,000.

GeorgiaÂ’s Special Spinach Triangles (Spanakopitakia)



“Spanakopitakia is one of the most popular meze from Greece. It can be served hot or cold.”



Ingredients:



2 to 2 ½ pounds fresh spinach



1 cup olive oil



1 large onion, finely chopped



½ cup chopped scallions



½ cup chopped fresh parsley



½ cup chopped fresh dill



2 large eggs



½ pound feta cheese, crumbled



½ cup grated kefalotyri or Parmesan cheese



1 pound fillo dough



Olive oil, for brushing the triangles



Directions:



Preheat the oven to 350 degrees. Wash the spinach very well and drain it in a colander. When it has drained completely, chop the spinach very thin. In a large saucepan, heat ½ cup olive oil and sauté the onion, scallions, dill, and parsley for 5 minutes. Add the spinach and stir the mixture together. Lower the heat and let simmer for 10 to 15 minutes. Transfer the mixture to a colander to drain any liquid, then place spinach mixture into a large bowl and allow it to cool for 20 minutes. In a medium bowl, beat the eggs, stir in the feta cheese, sprinkle with the grated kefalotyri and a little salt and pepper, and mix the ingredients well. Unroll the fillo dough and keep it covered while working with 2 sheets at a time. Brush each one with oil, then stack one on top of the other. Cut the fillo into 5 long strips. Place 2 teaspoons of the spinach mixture in the middle bottom of each strip, then fold the right corner up and to the left, to form a right angle. Continue folding like a flag, making each into a triangle. Place the triangles 1 inch apart on a greased cookie sheet. Brush the tops with oil and sprinkle with a little water. Bake for about 25 minutes or until golden brown.

Baked Corn Pudding (Pure de Maiz)



Ingredients:



6 tablespoons butter



1 large onion, chopped



1 garlic clove, minced



½ of a green bell pepper, chopped 



½ of a red bell pepper, chopped



½ of a jalapeno or another hot pepper, chopped



4 tablespoons flour



Salt and pepper to taste



2 cans of cream style corn



6 eggs, slightly beaten



2 cups milk



Directions:



Melt the butter in a skillet at medium heat. Add onions, garlic, and all 3 peppers. Cook, stirring until vegetables are tender. Stir in flour, salt, and pepper, until mixture is bubbly. Remove from heat. Add corn, eggs, and milk, stirring the entire time until the mixture is well blended. Pour the corn mixture into a shallow, buttered bowl or baking dish. Bake in oven at 350 for 55 minutes or until center is set.

Grandma DeMasiÂ’s Peppers and Tomatoes Al Gennaro



Ingredients:



2 tablespoons olive oil



2 garlic cloves, chopped very fine



3 ½ cups fresh tomatoes, cored, blanched, peeled, and chopped coarse (or canned plum tomatoes)



1 teaspoon salt



1 tablespoon chopped, fresh basil (or 1 teaspoon dried)



½ teaspoon sugar



1/3 cup olive oil



1 large Italian sausage (1/4 pound), cut on a slant into 1-inch slices



2 pounds green peppers, seeded and quartered



2 onions, sliced (about 1 cup)



Pepper, ground fresh, to taste



1/3 cup dry red wine or Madeira (optional)



Directions:



Heat 2 tablespoons olive oil in a large skillet; add garlic and sauté until lightly browned. Add tomatoes, salt, basil, and sugar. Cook over medium heat for 30 minutes. While tomatoes are cooking, heat the remaining 1/3 cup olive oil in another skillet, and brown the sausage slices well on all sides. Transfer the slices with a slotted spoon to the tomato mixture in the other skillet.



Reserve the oil in which the sausage cooked in the skillet. Add peppers to same skillet and toss them well. Sauté for about 8 to 10 minutes, then add the onions, and cook for another 20 minutes, or until vegetables are softened and slightly charred. Add them to the tomato mixture, and cook together for an additional 5 to 10 minutes. Adjust seasonings to taste. Add wine or Madeira (if you wish) to the skillet after the peppers and onions have cooked. Deglaze the pan, and add the liquid to the tomato and pepper mixture before the last 5 or 10 minutes of cooking.

Venture Capital Investment Swings Back Up to $453 Million



CapitalHunter. com was able to verify that $453 million of venture capital was invested in 40 U. S.-based companies during the past week.

Weekly funding has oscillated around the $400 million mark the past couple of months, and this week was no exception. Venture capital investment rose once again this week, as over $453 million was placed into 40 companies, a week-over-week rise of 36% from last week’s $333 million. Unlike the past two weeks, where software and biotechnology investment dominated the headlines, this week’s funding represented a more egalitarian distribution between the sectors, as no industry received more than 18% and seven sectors received more than 5% of the $453 million pie.



Healthcare services investment was skewed, however, by the $77 million investment received by Take Care Health Systems, a provider of retail-based healthcare delivery systems based in Conshohocken, Pennsylvania, which was the largest investment in a healthcare services company in over two years. The funding also accounted for the totality of healthcare services funding this week. Other notable investments for the week include ISE, a San Diego based provider of hybrid electric drive systems for trucks and busses which received a $25 million Series B investment from the trio of NGP Energy Technology Partners, Rockport Partners and Third Avenue Management, the Regency Beauty Institute, a Brooklyn Park, Minnesota based provider of cosmetic education which received a $20 million investment from LLR Partners and Prairie Capital, and Luxtera, a San Diego based company that provides IC technology to the optical communications industry which received a $22 million Series C investment from a consortium of investors, including Cal Tech, Sevin Rosen Funds, New Enterprise Associates and August Capital.



IPO Activity



IPO activity showed an uptick, as three companies listed this week, up from one last week. The three companies are as follows:



Alexza Pharmaceuticals (NASDAQ:ALXA) priced at $44 million and is a provider of therapeutics for the treatment of acute and intermittent conditions.



Eagle Test Systems (NASDAQ:EGLT) priced at $100.8 million and is a provider of high-performance automated test equipment for the semiconductor industry.



Tam SA (NYSE:TAM) priced at $463.8 million and is a Brazilian provider of air transportation services.



Top Venture Capital Investments



Take Care Health Systems, a provider of retail-based healthcare delivery systems, received $77 million in early stage funding from Beecken Petty O'Keefe and others.



Coremetrics, a provider of online marketing analytics services for the capture of clickstream activities, raised $31 million in Series D financing from Accel partners, FTVentures and Highland Capital Partners.



ISE raised $25 million in Series B funding from a consortium of private equity firms listed above.



Funding Breakdown



California was the largest recipient of venture capital funding this week. Fourteen California companies raised $186.8 million in new equity financing, which accounted for 41% of the total venture capital invested this reporting period. The largest private venture financing in California was placed into Coremetrics.



The healthcare services sector raised the most venture capital this reporting period, but the entirety of the funding was placed into Take Care Health Systems, mentioned in the introduction above. Software, the second most popular sector this week, had fourteen companies raise $72.9 million in venture financing, which accounted for only 16% of all investment this period. The largest private venture financing in the software sector this period was placed into BlueNote Networks. Bluenote Networks received $15 million in Series B funding and is a provider of IP telephony software and services.



Start-ups raising their Series A round of financing represented the largest share of the funding pie, as 34% of all money invested this period went into such companies. The largest such financing was placed into the aforementioned Take Care Health Systems.



Summary



A total of 98 equity financings of private and public companies occurred during this reporting period. Each of the 98 equity financings are profiled in a weekly (in this case biweekly) report which include venture financings, private placements, secondary offerings, PIPE’s, and IPO’s.

This New Years, North Americans can pledge to make a dent in the mountains of waste right in their own communities.



How can we, as individuals, participate in waste management? Because some of us are so overwhelmed with Earth’s problems, we feel that our contributions have no real consequence in the end. For others, social barriers can be an issue. A lady we once knew confessed that she did not want to be seen buying used items or being concerned with power use. She was worried people would see her as cheap – a scrooge – when the family was so affluent. Yet, she was very careful to be seen with recycling bins out on the curb on pick-up day, because that was thought to be the thing to do socially.



Now is a good time to put an end to these negative thoughts and feelings of false pride. Waste reduction is not about ego - it is about the health of the planet and of our nation. Communities would be wise to look at Nova ScotiaÂ’s waste reduction success and try to emulate it in their area. With the highest waste reduction rates in Canada, Nova Scotia has reduced landfill contributions by 46% - saving about $31 million per year - simply by making the most of the organic and recyclable materials. Curbside recycling service (Blue Box) is available to 99% of its residents and 76% now enjoy curbside organic service (a. k.a. Green Box).



With better management of organic and recyclable waste, individuals do not have to put the trash out as often because the odors and volume are greatly decreased. This also means that we are consuming less plastic garbage bags. As an additional fiscal incentive, many garbage collection companies offer discounts to homes with reduced waste.



We can also help the waste management industry run more efficiently. For instance, when only full garbage bags and Blue or Green boxes are put out on the curb, the garbage truck does not have to stop as often and burn fossil fuels inefficiently while idling. (Incidentally, vehicle idling is responsible for 3% of the air pollution problem.) Similarly, by collapsing boxes before recycling we are ensuring that space is used more efficiently, thereby reducing the number of bins needed for transporting materials.



Recycling, alone, has a huge impact on the environment. A study of a 100-unit apartment building practicing maximum recycling found it would save 21.93 thirty-foot trees, 26.86 cubic yards of landfill space, 8,389 kilowatts of electricity, and 77.4 pounds of air pollution in just one year!



So you see, these seemingly small choices and efforts towards waste management really do make a difference.

J., and Specialist Michael King, of Philadelphia, visited Health Advocate’s Plymouth Meeting headquarters on August 4th to accept the gifts. The Plymouth Meeting-based unit, currently stationed in Iraq, received the boxes on August 10th.



“We wanted to honor the sacrifice and bravery of our armed forces, because they do so much to protect the values we enjoy everyday,” said Martin Rosen, Executive Vice President and cofounder of Health Advocate. “The fact that the unit is based just a couple of miles from our headquarters just made it a natural decision.”



The idea for the collection was spawned by Ed Hampson, a Senior Health Advocate with the company, whose son John is currently stationed in Iraq with the 1st Battalion of the 111th Infantry. The collection drive, which was planned and supervised by an activity team of Health Advocate employees (Sue Clark, Sarah Forsyth, Betty O’Reilly, Eva Zuckerkandel, and Shauna Duffin), began on July 2 and ran through July 15. Health Advocate employees were encouraged to donate supplies and money as part of a company-wide effort.



“The level of participation reflected the commitment of our employees to support our troops and the local men and women from our community who are serving our country, and that was quite wonderful,” Rosen said. “The troops deserve it.”

Privately held Thai biotech company, Immunitor Corporation Co., Ltd, has submitted to the Thai FDA the application for Phase III clinical trial. The application is based on pending investigational R&D drug permit that was granted in 2000. The primary endpoint of the trial is aimed at confirming the beneficial property of V-1 Immunitor (V1) shown for AIDS-associated wasting. When approved, the study will be carried out by the clinical team of Dr. Orapun Metadilogkul, the President of Occupational and Environmental Medicine Association of Thailand at Rajavithi General Hospital in Bangkok – the largest public hospital of the Ministry of Public Health.



Recently published retrospective, large-scale study in 650 AIDS patients demonstrated that V1 was safe, effective, and highly promising as an inexpensive therapy for the management of opportunistic infections and AIDS-associated decline in body mass (http://www. nature. com/nature/view/040122.html#6 (http://www. nature. com/nature/view/040122.html#6)). The probability value obtained in this study (P=0.00000000000000065) indicates that obtained results were non-random and highly significant statistically. These findings support the results of open label phase I study published two years ago and of placebo-controlled phase II study completed last year.



V-1 Immunitor is an orally available therapeutic AIDS vaccine, which has been licensed in Thailand and certain countries in Africa as a dietary supplement. The vaccine elicits mucosal immune response in the gut, which appears to result in reversal of weight loss in 76% of AIDS patients. Several published clinical trials have shown that V1 is safer and cheaper than standard antiviral therapy.



The Phase III trial seeking licensure for AIDS-related wasting represents a "streamlining" of the Company's approval strategy. Obtaining drug label for such an indication is a stepping stone for Immunitor’s ongoing research program aimed at developing effective and safe vaccine for treatment and prevention of HIV."



Principal Investigator, Dr. Aldar S. Bourinbaiar, the Scientific Director of the Company, said, "Our results compare very favorably to any published results of treatment for this indication. The results we have seen are the best I've observed with this difficult-to-treat condition, which is associated with certain death. Currently available nutritional supplements or cocktail antiviral drugs known as HAART are inefficient for correction of wasting and V1 can be useful add-on option to HAART or as stand-alone therapy. Other treatment options like recombinant human growth hormone (HGH), produced by Swiss company Serono, are quite effective but they are too expensive for third world countries.”

In addition to its compactness, the iMRI also provides exceptional image quality. This image quality lets clinicians plan the best approach for reaching tumors and lesions. It also enables surgeons to more clearly see tumors and lesions, as well as the margins between healthy and unhealthy tissue. With this clearer visualization, and real-time imaging, surgeons can confirm that a tumor has been removed, safely and entirely, while patients are still in the operating room, rather than subjecting patients to additional operation for residual tumors that are detected in a post-operative MRI.

For AlaVie co-founders Heather Glenn and Mary Beth Gonzales, who both lost their mothers to cancer within the last two years. this latest fundraiser is a continuation of their efforts to remember their loved ones and help others embrace a healthy lifestyle.
