Report Predicts eHealth Will Unlock The Healthcare Outsourcing Market.



A report by Cambridge based ehealth consultants, Wireless Healthcare, points to a number of key drivers it believes will bring about an increase in the use of outsourcing within the healthcare sector. However it also warns companies they face significant resistance from clinicians, health service management and, in some cases government departments. This will be especially true where clinical processes are relocated overseas.

A report by Cambridge based ehealth consultants, Wireless Healthcare, points to a number of key drivers it believes will bring about an increase in the use of outsourcing within the healthcare sector. However it also warns companies they face significant resistance from clinicians, health service management and, in some cases government departments. This will be especially true where clinical processes are relocated overseas.



In the early 1990's demonstrations of ehealth applications usually involved an x-ray being transmitted from a remote part of the developing world to a hospital in Europe or the USA. A clinician would eagerly relay a diagnosis to the patient's local hospital. These systems were unsustainable and collapsed when funding dried up. However, soon, the flow of x-ray images could be in the other direction - from hospitals in the west to diagnostic centres in the developing world. While, this time, the business case is sound clinicians are less enthusiastic.



While clinicians fear outsourcing will lead to a loss of skills within the UK, Wireless Healthcare suggests that cost savings would free up resources and allow the NHS to concentrate on core competencies such as surgery. At present surgery is being outsourced from overseas as patients arrange their own operations in other European countries.



Wireless Healthcare warns outsourcing companies not to play down the low cost of their services in the hope of overcoming resistance from clinicians. Claims by outsourcing providers that they are merely exploiting their geographic location to provide services out of UK working hours are unlikely to placate the fears of health workers who are well aware of the impact of outsourcing on the IT sector. At the same time understating potential savings will discourage potential early adopters of offshore outsourcing within the private healthcare sector.



The report suggests that outsourcing companies may also face a degree of resistance from the government. As one of the largest employers in the UK, the NHS is a useful tool for channelling government spending into the economy. Spending on the health sector finds its way into other sectors of the economy well within the four-year electoral cycle. It is therefore in the government's interest to forgo the savings (estimated to be up to 40%) of outsourcing work overseas and, instead, bring health workers to the UK where the bulk of their wages are spent within the UK economy. The NHS recently decided not to include a proposal, from the India based outsourcing company Wipro, in its current IT planning.



There is, according to Wireless Healthcare, a compelling argument for offshore outsourcing in that it provides access to a large number of people of working age in countries such as India - where the demographic profile is the mirror image to that of Europe.



Wireless Healthcare believes the impact on countries providing outsourced healthcare will be neutral. At first sight outsourcing appears to offer an alternative to the wholesale recruitment of medical staff by the NHS. However medical workers employed in diagnostic centres run by outsourcing companies are just as inaccessible to local patients as medical staff who have moved overseas. The report, does though, point out that both direct recruitment and employment within outsourcing companies will encourage young people to train for a career in medicine which in turn will increase the pool of medical workers available to local health providers.



The report predicts that in the long term pressure from a growing number of retired people for lower cost healthcare will force the government and the NHS to seriously consider outsourcing. At the same time a number of private health initiatives built around outsourcing may gain popularity and bring about a 'creeping privatisation' of the UK healthcare sector.



According to Peter Kruger, a senior analyst with Wireless Healthcare, "In a globalised market it matters little where in the world a product or service is made available, eventually it will find a channel to the consumer. Healthcare seems to be one of the last sectors of the economy to wake up to this fact."



Wireless Healthcare sees an analogy between outsourced manufacturing of consumer electronics and outsourced clinical processes. As Peter Kruger points out. "Just as the widespread ownership of personal computers was made possible by low cost electronics manufacturing in the Far East so universal access to medical services will become increasingly dependent on clinical processes outsourced from offshore medical centres."

Reflecting economic conditions in the marketplace, the average salary adjustment for staff positions in nonprofit organizations decreased marginally from 4.0% to 3.8% between 2007 and 2008. However, this may not account for the significant downturn in the U. S. economy from September through December of 2008. According to a new study released by Total Compensation Solutions (TCS), financial conditions in the economy are reflected in the level of charitable contributions received by nonprofit charitable organizations and this, in turn, may impact the salary budgets for those organizations. This suggests that nonprofits may need to take decisive action in either cutting back staff or reducing the salary budgets for the upcoming year.



The 2008/2009 Not-for-Profit Compensation Survey, conducted by TCS, compiles data on 73 unique positions found in over 530 nonprofit organizations. In previous surveys, we observed that nonprofits were changing the way they pay their employees. Specifically, we observed an increase in the number of organizations that were using formal bonus/incentive compensation plans for their staff. For 2008, we presume that these same organizations will either pay significantly lower bonuses or suspend bonus payments altogether. We believe that this is a reaction to declining economic conditions throughout the economy. 



This year's study of compensation among nonprofits reports average pay levels for executives in mission critical and support departments among these organizations. An overall summary of the average total compensation for executive positions is attached to this press release



The study observes that salary adjustments for 2008, turnover, bonus/incentive amounts were all tracking the practices of for profit companies for most of the year. However, the study identifies compensation practices for the first part of 2008 and does not account for moderate changes in revenue stream, charitable contributions, and prospective planning for 2009 which may be changing even as this document goes to press. "If you could break out the first six months of 2008, nonprofits had a pretty good year," says Paul Gavejian, Managing Director of TCS. "Our clients in the nonprofit sector were on track for record contributions, their revenue generating divisions were making money; and their employees were providing excellent response to their governing communities…then the financial crisis hit the economy and everything changed." 



"Preliminary salary budget reports suggest that both for profit and nonprofit organizations are considering either a freeze on salary adjustments or minimal increases." Historical trends indicate a high degree of correlation between salary budgets and the consumer price index (CPI). "With a significant increase in the unemployed labor force and a lower CPI, many employers are simply being cautious about increasing pay," according to Gavejian.

After Hip Resurfacing, Cleveland Fire Battalion Chief, 55, Wins 3 Scott Firefighter Combat Challenges



Painful hip arthritis had sidelined Donald Kempf, 55, until hip resurfacing let him return to competing in--and winning--the Scott Firefighter Combat Challenge®. William Stanfield, MD, of the Center for Orthopedics in Sheffield Village, Ohio resurfaced Kempf's left hip to relieve the arthritis pain and restore his normal hip function--without the movement restrictions of total hip replacement.

Like many Baby Boomers, Cleveland Fire Battalion Chief Donald Kempf, 55, was suffering from painful hip arthritis, but unwilling to give up his favorite sport--in his case, the Scott Firefighter Combat Challenge®.Then a firefighter colleague told him about hip resurfacing--an alternative to total hip replacement that allows many patients to resume active sports.



According to a study published in March 2010 by the American Journal of Sports Medicine, 82% of hip resurfacing patients felt no restriction while performing sports after their surgery. Since undergoing hip resurfacing, Kempf has won three Scott Firefighter Combat Challenge competitions--two of them within a year of his October 2008 surgery.



"Hip resurfacing allows you much more freedom of movement than total hip replacement," says board-certified orthopedic surgeon William Stanfield, MD, of the Center for Orthopedics in Sheffield Village, Ohio. "It recreates the normal hip anatomy rather than changing it, which is what a total hip replacement does.



"Hip resurfacing reshapes the head of the thighbone or femur and resurfaces it with a cobalt chromium metal cap, much like capping a tooth," Dr. Stanfield explains.



"With a total hip replacement, there are certain things you just can't do because of the risk for dislocation--where the ball pops out of the socket," says Dr. Stanfield, one of the first orthopedic surgeons in the U. S. to perform hip resurfacing. "After hip resurfacing, you don't have those precautions--such as not crossing your legs or not bending your hips past 90 degrees."



"I've always been athletic," says Kempf. "I first competed in the Firefighter Combat Challenge in 1994, and have won my age division in regional competitions almost every year since. But when my left hip started hurting me, I thought my athletic life was done. I spent a whole year in agonizing pain, just limping around.



"I was kind of afraid of surgery," Kempf admits. "My father has had five hip replacements, starting when he was my age. I knew about traditional hip replacement--you can still stay active, but it's more like golf and walking around and things like that. I wasn't quite ready to give up my crazy stuff."



Kempf's "crazy stuff"--the Scott Firefighter Combat Challenge--has been featured on ESPN and Versus. In that competition, pairs of competitors, wearing "full bunker gear" and breathing apparatus, race head-to-head as they simulate the physical demands of firefighting by performing a linked series of five tasks, including climbing a five-story tower, hoisting, chopping, dragging hoses and rescuing a life-sized 175-pound "victim."



"If you perform that the way you should on an actual fire run, you're supposed to pace yourself," says Kempf, who has placed as high as third in the world in the over-40, over-50 and chiefs divisions. "You could probably walk through this and finish it in five to seven minutes. But as the competition grew, the times got down to around two minutes and some of the world-class guys are at about a minute and a half. You get done with this and you pretty much can't even stand up, you can't breathe. People actually catch you at the finish line--they hold you up.



"When I found out about hip resurfacing, and the fact that you could have the procedure and remain athletic, I went for it," says Kempf.



"I told Mr. Kempf before the surgery that I couldn't make him any promises, but hip resurfacing would be the only way he would have a chance to get back to doing the firefighters' competition," Dr. Stanfield recalls. "There wouldn't be any way to do that with a full hip replacement."



"The day of surgery, my hip pain was gone," says Kempf. "I was back to work in six weeks--and back to my training. By the following August, 10 months after surgery, I won the over-50 portion of the Firefighter Combat Challenge in Windsor, Ontario, Canada. A couple of months later, I won in my age group at a competition in Indiana as well."



Kempf also recently won a competition in Milford, Ohio, near Cincinnati. "I'm planning to participate in one or two more regional competitions this summer--and then in the World Championships this November in Myrtle Beach, SC," he says.



"Not everyone is a candidate for hip resurfacing," notes Dr. Stanfield. "It's primarily indicated for younger patients. They don't have to be high-performance athletes--some patients are mechanics who need the freedom of movement to get under cars, or electricians who have to climb around and re-wire air conditioners."

"At first, many near-death experience patients are confused and fearful because the experience is so out of their realm of awareness and so uncommon to their everyday senses", notes course co-author Dr. Harris Friedman, clinical psychologist and Research Professor in the Counseling Psychology Program at the University of Florida. Not only do they lack the words to describe the event, but they lack the ability to integrate the NDE into their personal life history. Moreover, this confusion and fear may close off any future attempts to discuss the NDE with their healthcare providers. This is significant because the short - and long-term effects of NDE have the potential to negatively impact a patient's emotional, physical/physiological, social, and spiritual life, at times, requiring clinical intervention.
